29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    Employee(s) using the pallet wrap machine on the east side of the shipping department were exposed to electrical hazards due to the power cord for the pallet wrap machine being frayed.  The outer jacket of the power cord were damaged approximately 4 inches in length, thus exposing the live conductors.
