29 CFR 1904.29(b)(1): In the food handling and service areas at Laredo Medical Center, on or about January 10, 2013, and at times prior thereto, the employer did not correctly record the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2012: In the food handling and service areas at Laredo Medical Center, on or about January 10, 2013, the employer did not correctly record the following work-related injuries or illness on the OSHA Form 300 or equivalent for the calendar year 2012: a. On or about August 4, 2012: a utility employee working in the dish room was listed on the 300 form, but the description of the injury or illness states no injury reported. b. On or about October 31, 2012: a cook working in the kitchen production area received irritation and/or chemical burns to both hands and the employer did not enter the number of days away from work in column H. c. On or about November 6, 2012: a utility employee working in the dish room experienced a cut to the hand and the employer did not enter a description of the injury in column F. d. On or about November 12, 2012: a food service employee working in the cafeteria dining room, stacking tables, was struck on the neck, back, and/or head and the employer did not enter a description of the injury in column F.

On or about November 12, 2012, a food service employee stacking tables in the cafeteria dining room was struck by a falling table on the neck, back, and/or head.
