On or about November 6, 2012:   a utility employee working in the dish room experienced a cut to the hand and the employer did not enter a description of the injury in column F.    d.  On or about November 12, 2012:  a food service employee working in the cafeteria dining room, stacking tables, was struck on the neck, back, and/or head and the employer did not enter a description of the injury in column F.
