29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk was not guarded: In the Kitchen in front of the incoming dish/tray conveyor, the employer did not protect employees from floor drain holes which measured approximately ten and one-half inches in width and one and one-half inches in depth.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: In the Kitchen, on the Hobart dishwashing machine, employees removed jammed dishware from the machine by gaining access through inspection doors on the side of the machine. The inspection door on the center section nearest the loading end was missing the latch hook, which held open the door. This condition exposed employees to struck-by hazards.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: In the Kitchen at the dishwashing area, employees received, rinsed, loaded and unloaded dishes from and onto conveyors where the chain and sprocket on the motor was unguarded such as but not limited to: a) the dirty dish conveyor to the rinsing area; and b) the dryer end of the Hobart dishwasher.
