29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: In the Kitchen, on the Hobart Dishwashing machine, during maintenance employees used the inspection doors to gain access to the inside of the dishwasher and kitchen employees removed jammed dishware from the machine. The inspection door on the center section nearest the loading end was missing the latch hook which held the door open. This condition exposed employees to struck-by hazards.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: In the Kitchen at the dishwashing area, maintenance personnel maintained all equipment where kitchen employees received, rinsed, loaded and unloaded dishes from and onto conveyors where the chain and sprocket on the motor was unguarded such as but not limited to: a) the dirty dish conveyor to the rinsing area; and b) the dryer end of the Hobart dishwasher.
