29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.      Jobsite:  Second floor    Scaffolds were accessed from access points that were more than 2 feet below the scaffold.

Instances:    a)  Unguarded window openings on second floor,    b)  Unprotected floor edges on second floor,    c)  Unprotected floor edges in the attic, and    d)  Employee working on garage gabled wall without being tied off.    BUILD RIGHT SERVICES CORPORATION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501 (b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 316106517, CITATION NUMBER 1, ITEM 4, ISSUED ON JANUARY 25, 2012 WITH A FINAL ORDER DATE OF FEBRUARY 15, 2012.
