29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. East side of the building; employees walk/work area was obstructed with bricks, debris, scrap wood, protruding nails and soil piles in and around work areas, exposing employees to tripping hazards.

29 CFR 1926.403(a): Approval.In front of the Superintendents trailer; the employer used a repaired flexible cord that was not returned to its original state and approved by a qualified electrician, exposing employees to electrical hazards.
