29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.    East side of the building; employees walk/work area was obstructed with bricks, debris, scrap wood, protruding nails and soil piles in and around work areas, exposing employees to tripping hazards.

All electrical conductors and equipment were not approved; repairs did not return the equipment to the state in which it was initially approved.
