29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) 2500 University Place NW Job Site - An employee was not provided adequate hazard recognition training as noted where the employee was using a Makita portable electric hammer drill, model number HM1810, to break up an existing concrete slab and not using a ground fault circuit interrupter or an assured equipment grounding conductor program. Also the plug cap for the Makita hammer drill was for a three wire type service and the cord for the hammer drill was a two wire service, the temporary electric receptacle was missing a cover plate exposing live electrical parts, and the outlet was not mounted and hanging by its service conductors, 120 volt, 15 amp service, on or about 01/08/2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
