29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).      On or about January 10, 2013, on the south side, an employee was exposed to a fall hazard of approximately 10 feet while engaged in framing activities on a residential construction site.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided.      On or about January 10, 2013, on the south side, the employer did not provide an extension ladder where the employee could access and egress the upper landing exposing the employee to a fall hazard up to 10 feet.
