29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift: a) Spider Enterprises, LLC on or about January 10, 2013, at the address of 20750 Midstar Dr. Bowling Green, OH: The employer failed to ensure employees working from aerial lifts were adequately protected from falls. Employees working from the lifts were not clipped off to the basket while the platform was elevated.
