The platform was missing a plank. An employee was installing insulation from the scaffold approximately 18 feet above grade, observed on or about 1/2/13.  (b) Jobsite located at Brandywine Senior Living, 311 Route 73, Voorhees, NJ, Pod, and 2nd Floor Deck: The scaffold erected on the 2nd floor deck of the pod was not fully planked, the platform was missing a plank. (b) Jobsite located at Brandywine Senior Living, 311 Route 73, Voorhees, NJ, SW Wing:  A ladder was not provided for the employee to use.  The employee was erectingscaffold from the 3rd level side bracket.  The employee climbed the cross braces to get to that side bracket, observed on or about 1/31/13.  (c) Jobsite located at Brandywine Senior Living, 311 Route 73, Voorhees, NJ, Exterior Pool Room Wall: The ladder provided for use was only used to access the first level of the scaffold. The employee climbed the crossbraces to access the 2nd and 3rd levels, observed on or about 2/22/13.
