29 CFR 1926.451(e)(8): Direct access to or from another surface was not used when the scaffold was more than 14 inches (36 cm) horizontally and 24 inches (61 cm) vertically from the other surface: (a) Noron, Inc. on or about January 10, 2013, at the address of 20750 Midstar Dr. Bowling Green, OH: The employer failed to ensure a safe means of access/egress was provided to employees. During mechanical work, employees were utilizing a JLG model 3394RT scissor lift to gain access to an elevated work area. Employees were required to step more than 42 inches vertical from the scaffolding platform to access the working area. These employees were exposed to a 35ft. fall to the ground below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Noron, Inc. on or about January 10, 2013, at the address of 20750 Midstar Dr. Bowling Green, OH: The employer failed to ensure employees installing exhaust fan units atop a commercial structure were adequately protected from falls. The employees working in this condition were exposed to an approximant 35 ft. fall to the ground below.
