29 CFR 1910.134(c)(1):   A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:   On or about January 10, 2013:  The employer had not established a written respiratory protection program.  The employer requires the employee spraying the bedliner material to wear a half-face tight fitting respirator.  The bedliner material contains 4,4-Diphenylmethane Diisocyanate.

29 CFR 1910.134(e)(1):   The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:   On or about January 10, 2013:  The employer did not ensure that the employee working in the bedliner spray booth had an annual medical evaluation prior to wearing the 3M half-face tight fitting respirator.  The employer requires the use of the respirator when spraying the bedliner material.

29 CFR 1910.134(f)(1):   The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):   On or about January 10, 2013:  The employer did not ensure that the employee working in the bedliner spray booth was fit tested prior to using the 3M half-face tight fitting respirator he is required to wear when spraying the bedliner material.  The bedliner material contains 4,4-Diphenylmethane Diisocyanate.

Employees are exposed to chemicals and materials such as but not limited to isocyanates, acetone, and flammable liquids N.O.S. (brake cleaner).
