29 CFR 1926.451(c)(2)(v):   The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  At the north entrance of a commerical building under construction, an employee was elevated on a wooden pallet which was not secured to the forks or carriage of the JLG rough terrain fork lift truck. The employee was exposed to a fall of approximately 16 feet.

At the north and south end of the exterior of the building under construction, several employees were observed inserting screws to sheet metal decking approximately 19 feet above ground level without the benefit of fall protection.
