29 CFR 1926.501(b)(1):  Employee(s) were performing work on a walking/working surface with unprotected side or edge which was 6 feet (1.8 m) or more above a lower level and were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a) 2416 Willow Street, Lancaster PA - Employee(s) were engaged in removal of debris from the second floor of the building being demolished and were not protected with a fall protection system, thereby exposing employee(s) to a fall of 8 to 16 feet, on or about January 14, 1013.     (b) 2416 Willow Street, Lancaster PA - An employee was standing at the edge of the first floor handing down tools to an individual in the basement of the building being demolished and was not protected with a fall protection system, thereby exposing employee to a fall of 8 feet, on or about January 14, 1013.   No abatement certification or documentation required.

29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations:    (a) 2416 Willow Street, Lancaster PA - An engineering survey was not available in writing by a competent person prior to employees(s) entering the building to ensure the structure could sustain the weight and operating forces imposed from the demolition to the floors and walls, thereby exposing employee(s) to a structure collapse, on or about January 14, 2013.     No abatement certification or documentation required.

When the presence of any such substances is apparent or suspected, testing and purging was not performed and the hazard eliminated before demolition was started:    (a) 2416 Willow Street, Lancaster PA - The employer did not determine if any types of dangerous substances, such as but not limited to asbestos and lead, were present and eliminated before demolition work started, on or about January 14, 2013..
