OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: On or about January 11, 2013, employees were working on Rig # DD12 at a known hydrogen sulfide field; without using the H2S monitoring system. This condition exposed employees to potential hydrogen sulfide gas.

29 CFR 1910.23(a)(1): Every stairway floor opening shall be guarded by a standard railing constructed in accordance with paragraph (e) of this section: On or about January 11, 2013, employees were working on Rig # DD12 where the main stairway to the rig was missing its guardrail. This condition exposed employees to potential falls of up to approximately 4.5 feet.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder: On or about January 11, 2013, employees were working on Rig #DD12 where the platform was missing guardrails on either end of the rig. The platform measured at approximately 4.5 feet in height.
