29 CFR 1910.332(b)(3):  Qualified persons (i.e. those permitted to work on or near exposed energized parts) were not trained in and familiar with all of the requirement in 29 CFR 1910.332(b)(3) through 29 CFR 1910.332(b)(3)(iii):   a) Establishment: On or about 1/16/2012, employees were permitted to perform testing to assure that the equipment to be worked on is de-energized, using a fluke meter, and had not been provided with training required by 29 CFR 1910.332(b)(3) through 29 CFR 1910.332(b)(3)(iii).

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  a) Establishment:  On or about 1/16/2013, an employee was using a fluke meter to check that various electrical equipment to be worked on was not energized and had not been provided with electrical protective equipment, such as but not limited to, gloves, clothing for arc flash protection, faceshield and safety glasses.
