29 CFR 1926.404(b)(1)(ii):  On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:     (a) Jobsite - on or about January 14, 2013 - electric tools that were plugged into a dual phase portable generator were not protected by a Ground Fault Circuit Interrupter (GFCI).

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) Building Roof - on or about January 14, 2013 - conventional fall protection was not available for the employees that were installing roofing materials on a seven on 12 pitched roof deck.  Ground to eave elevation was approximately 13 feet.

29 CFR 1926.405(g)(2)(iii):  The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used:    (a) Jobsite - on or about January 14, 2013 - a 12-3 SJTW electrical extension cord had a makeshift splice.  Insulation qualities and outer sheath properties were not retained.
