At 221 Baker Ave, Great Bend, KS in the loadout area, the elevated wood passageway adjacent to the aggregate storage bin has cracked and deteriorated wood planking, exposing employees to falls of approximately 30' to the ground below.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service:    At 221 Baker Ave in the shop area, the employer failed to removed damaged flexible cords from service, exposing employees to electrical shock in the following instances:    1)  The Hobart wirefeed welder had an attachment plug with wire sheathing pulled free from the plug.    2)  The Leviton extension cord has sheathing pulled free from the plug.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein:    At 221 Baker Ave, Great Bend, KS in the load out area, two 1500 gallon tanks had no labels to identify the contents, exposing employees to calcium chloride, which may cause eye injury or nausea if ingested.
