Employees engaged in chipping activities inside truck concrete mixers  were exposed to health hazards in that the employer had not determined, through a medical evaluation, the employee's ability to use a respirator before the employee is fit tested or required to use a respirator.

Employees engaged in chipping operations inside concrete mixers on the trucks were exposed to health hazards in that the employer had not determined, through a qualitative fit test (QLFT) or quantitative fit test (QNFT), the respirators effectiveness before the employee is required to use a respirator.

29 CFR  1910.134(k):The employer did not provide comprehensive, understandable training which did not occur annually and/or more often if necessary:  Employees were exposed to health hazards.  Employees engaged in chipping activities were exposed to hazards in that they were required to wear respirators and the employer had not provided employees with comprehensible and understandable training. This training should include: Why the respirator is necessary and how improper fit, usage, or maintenance can compromise the protective effect of the respirator;  What the limitations and capabilities of the respirator are; What the procedures are for maintenance and storage of the respirator; How to recognize medical signs and symptoms that may limit or prevent the effective use of respirators.
