29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided:    a) Job site, rear of 2821 and 2823 Penn Avenue, Pittsburgh, PA: On or about January 11, 2013, during masonry activities, employees were not provided with a safe means of accessing or egressing the main roof areas where the worksite was located. Employees had to climb up on the rails of a deck, then across a gap between the rails and a nearby lower roof to gain access to a ladder located on it, in order to get onto the main roof.
