29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about 1/15/13, in the Mechanical Garage under construction on jobsite at 2240 Southwestern Blvd., West Seneca, NY: An employee used a DeWalt DW268 Screwdriver to fasten steel ceiling panels. The screwgun was connected to 120-volt ac power via a series of extension cords, the first of which was plugged into a temporary receptacle inside the building. The plug on the last extension cord in the series lacked a grounding prong. NO ABATEMENT CERTIFICATION REQUIRED
