29 CFR 1910.37(b)(4):  Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:      a) Department of Transportation Maritime Administration (MARAD) - Point Alameda Warehouse - On or about January 15, 2013, and at times prior thereto, directional exit signs were not posted indicating the direction(s) to the nearest exit when the direction of travel was not immediately apparent from:    a)  The rows of storage shelves in Bay 1.     b)  Office area into the main entrance access corridor.    c)  Bay 2 east caged area where the exit is located in a recessed area.

29 CFR 1910.178(a)(4):  Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly:      a) Department of Transportation Maritime Administration (MARAD) - Point Alameda Warehouse - On or about January 15, 2013, and at times prior thereto, the Mitsubishi Model FG40 Powered Industrial Truck with SN AF29 - 50204 was used with Vestil fork extensions and there was not written approval by the manufacturer.  The vehicle capacity, operation and maintenance instructions plates, tags and/or decals did not contain information based on use of the extension forks.

29 CFR 1910.178(a)(5):  If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:    a) Department of Transportation Maritime Administration (MARAD) - Point Alameda Warehouse - On or about January 15, 2013, and at times prior thereto, the Mitsubishi Model FG40 Powered Industrial Truck with SN AF29 - 50204 was used with Vestil fork extensions and the truck was not marked to identify the attachments and show the approximate weight of the truck and the attachment combination at maximum elevation with load laterally centered.

29 CFR 1910.1200(g)(8):  The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace:      a) Department of Transportation Maritime Administration (MARAD) - Point Alameda Warehouse - On or about January 15, 2013, and at times prior thereto the MSDS sheets for diesel and gasoline were not maintained as part of the MSDS program.  Diesel and gasoline are used in the powered industrial trucks used in the warehouse.
