29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: The employer does not ensure that lockout or tagout devices are affixed to each energy isolating device. This violation occurred on or about January 12, 2013, on the Delayed Coker Unit (DCU) where employees were exposed to thermal hazards when performing maintenance and service tasks without affixing a lockout or tagout devices on the hand wheels valves. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that lockout or tagout devices are affixed to each energy isolating device.
