The extension ladder that was used, did not extend at least 3 feet above the landing, on or about December 13th 2012.

The employees were not protected by any type of fall protection system, on or about December 13th 2012. Don Ritchie Roofing was previously cited for a violation of this Occupational Safety and Health Standard or it's equivalent 29 CFR 1926.501 (b) (13) which was contained in OSHA inspection # 312863483, Citation 01, Item 001, issued on 02/23/2010 with respect to a workplace located at 539 Glenwood Avenue in Palisades Park, NJ.
