29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Ibra Department and the Secondary Operations Department- The employer failed to ensure that all areas where hazards were created by the point of operation were guarded machinery such as the American Broach & Machine Company Broacher Machine, Serial #3225 in the Secondary Operators Department, and the #3 Chicago Rivet & Machine Company Riveter, Serial #722A-777-DS, in the Ibra Department.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) Secondary Operations Department- The employer failed to ensure that all points of operation on machinery such as, but not limited to the Rockwell Chop Saw, Serial #P225P4A7, were properly guarded to protect employees from the danger zone while performing cutting operations.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.      Phoenix Electric Manufacturing Company was previously cited for a repeat violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA Inspections Number 312473499, Citation 2, Item 2, and was affirmed as a final order on October 24, 2008, with respect to the workplace at 3625 N. Halsted St., in Chicago, Illinois.      Phoenix Electric Manufacturing Company was previously cited for a serious violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA Inspections Number 310113071, Citation 1, Item 2, and was affirmed as a final order on September 15, 2006, with respect to the workplace at 3625 N. Halsted St., in Chicago, Illinois.
