29 CFR 1910.67(c)(2)(iv):   The employer did not ensure employee(s) stand firmly on the floor of the basket and do not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position.    M/V "Brave Star"-    (a) Employee(s) working in the #4 cargo hold of the vessel were leaving and entering the basket while it was positioned at the top of the coaming opening approx. 50-55 feet above the bottom. While exiting the basket employee(s) were not tied off exposing them to falls.    (b) An employee operating the aerial lift device, repositioned the device to the other side of the #4 cargo hold against the ships ladder. At that time the employee disconnected his fall protection equipment and exited the basket and was exposed to falls of approx. 35 - 45 feet.

29 CFR 1910.67(c)(2)(ii):   An employee(s) operating an aerial lift(s) were not properly trained:    M/V "Brave Star"-  (a) Employees operating and working from the JLG Lift 600S aerial device were using the equipment improperly, in that they did not follow the manufactures operating and safety manual instructions as stated below;     Page 1 - 3 specifically states- "Do not place the boom or platform against any structure to steady the platform or to support the structure"    Page 1 - 3 Trip and Fall Hazards- "During operations, occupants in the platform must wear a full body harness with a lanyard attached to an authorized lanyard anchorage point."    Page 1 - 4 specifically states- "Keep both feet firmly positioned on the platform floor at all times".    Page 1 - 5 specifically states- "Use extreme caution when entering or leaving platform. Be sure that the boom is fully lowered.    Page 4 - 8 Shut down and park procedures state- "Use extreme caution when entering or leaving platform. Be sure that the boom is fully lowered."    Note: Abatement certification and supporting documentation are required for this item.

29 CFR 1915.131(h):   Compressed air hoses were not examined before use.M/V "Brave Star"-    (a) Employee(s) accessing the ladder way of cargo hold # 4 were exposed to a charged (125 psi) airline coupling that was missing its safety pin to prevent any whipping hazard in the event of becoming disconnected. The compressed airline was used in cleaning the cargo hold.     (b) Employee(s) operating the air wand in the JLG aerial lift located in cargo hold # 4 were exposed to a charged (125 psi) airline coupling that was missing its safety pin to prevent any whipping hazard in the event of becoming disconnected. The compressed airline was used in cleaning the cargo hold.     (c) Employee(s) working in the bottom of cargo hold # 4 were exposed to a charged (125 psi) airline coupling that was missing its safety pin to prevent any whipping hazard in the event of becoming disconnected. The compressed airline was used in cleaning the cargo hold.
