29 CFR 1904.41(b)(2):     The OSHA Annual Survey Form was not completed and sent to OSHA within 30 calendar days, or within the time stated in the Survey Form, whichever is later:    (a) Solid Waste Services of West Virginia failed to complete and send to OSHA, or its agent, the OSHA Occupational Injury and Illness Data Collection Form, 2011 within 30 days of the date Solid Waste Services of West Virginia received the Form, which was mailed to Solid Waste Services of West Virginia on or about July 22, 2012.    ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
