29 CFR 1910.147(c)(4)(i): Procedures were not documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 1/16/13 in the shop area; employees are performing service and maintenance ( installing and removing dies, product change-overs, set ups, threading mylar film through the die area ) on the Preco machine without using the ram stop blocks and locking out the energy isolation device. b) On or about 1/16/13 in the shop area; employees are performing service and maintenance ( installing and removing dies, placing make ready sheets ) on the Contech machine without documenting and utilizing a procedure for locking out the energy isolation device. c) On or about 1/16/13 in the Pleater room area; employees are performing service and maintenance ( cleaning the crimpers, performing set up and change overs ) on the mini-pleater machine without documenting and utilizing a procedure for locking out the energy isolation device.
