29 CFR 1910.22(d)(1):     In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official shall be marked on plates of approved design which shall be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate. Such plates shall not be removed or defaced but, if lost, removed, or defaced, shall be replaced by the owner or his agent.    Job site located at 4550 S 96th Street, Omaha, NE 68127-The employer did not ensure that the material stored in the upper storage area over the employee break room was safe by having the load limit per square foot for the area calculated and posted in the area on plates of approved design and securely affixed to the area.  The area had full 55 gallon drums of oil and other products stored in the area.

29 CFR 1910.178(l)(2)(ii):     Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    29 CFR 1910.178(l)(6):     Certification. The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.    Job site located at 4550 S 96th Street, Omaha, NE 68127-The employer did not certify that each forklift operater was evaluated as required by the requirements of paragraph l. The employer did not certify by including the name of the operator, the date of the training/evaluation. and the identity of the person(s) performing the training/evaluation. The employer did provide a video training session for operators and had them take a written test before giving them their license to operate a forklift, but did not conduct an evaluation and document the evaluation for each operator.

Each disconnecting means required by this subpart for motors and appliances shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.    Job site located at 4550 S 96th Street, Omaha, NE 68127-Electrical panels in the truck shop and next to the employee break room had breakers that were not karked to indicate the purpose of the breaker.
