29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors.    a. At the establishment, rear exit doors located at the North East and North West sides of the building were locked. Locks were key bolt type locks not designed for emergency egress exposing employees to injuries in case of an emergency, on or about 01/15/2013.

29 CFR 1910.133(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    a. At the site, employees use Rad Cure a curable coating during the printing process. Employees wear no eye or face protection exposing them to a splash hazard. The chemicals may caucuse injuries to eyes such as but not limited to cornea burns and blistering to the face and eyes. On or about 01/15/13.
