29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) South side of addition, west end of scaffold platform, on or about 1-16-13: One employee was working on an outrigger bracket scaffold platform that did not have an end guardrail, exposing the employee to a fall hazard of 14 feet 5 inches.

29 CFR 1926.451(g)(4)(xv): Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform and was 46 inches above the work platform. The endpoints at each upright were 41 inches apart: a )East side of building, on or about 1-17-13: Two employees were working on a scaffold platform without a mid-rail, using a cross brace at a height of 46 inches only, exposing the employees to a fall hazard of 13 feet 8 inches.
