While the cover is not in place, the pit or trap opening shall be constantly attended by someone or shall be protected on all exposed sides by removable standard railings. 29 CFR 1910.95(g)(1): The employer shall establish and maintain an audiometric testing program as provided in this paragraph by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  (a) Head saw stacker - On or about January 30,2013 employee was exposed to continuous noise at 278% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 97.4 dBA during the 432 minute sampling period. Exposure calculation included a zero increment for the time not sampled.  (b) Nailer - On or about January 30,2013 employee was exposed to continuous noise at 205% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 95.2 dBA during the 437 minute sampling period. Exposure calculation included a zero increment for the time not sampled.  (c) Saw operator - On or about January 30,2013 employee was exposed to continuous noise at 188% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 94.6 dBA during the 434 minute sampling period. Exposure calculation included a zero increment for the time not sampled.  (d) Nailer - On or about January 30,2013 employee was exposed to continuous noise at 144% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 92.6 dBA during the 434 minute sampling period. Exposure calculation included a zero increment for the time not sampled.  (e) Cutt off saw operator - On or about January 30,2013 employee was exposed to continuous noise at 112.1% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 90.8 dBA during the 433 minute sampling period. Exposure calculation included a zero increment for the time not sampled.  (f) Nailer- On or about January 30,2013 employee was exposed to continuous noise at 90.4% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 89.3 dBA during the 410 minute sampling period. Exposure calculation included a zero increment for the time not sampled.  (g) Resaw feeder - On or about January 30,2013 employee was exposed to continuous noise at 82.2% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 88.6 dBA during the 435 minute sampling period. Exposure calculation included a zero increment for the time not sampled.  (h) Resaw operator - On or about January 30,2013 employee was exposed to continuous noise at 79.7% of the permissible daily noise (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 88.3 dBA during the 434 minute sampling period. Exposure calculation included a zero increment for the time not sampled.

29 CFR 1910.215(a)(4):   Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.  (a) Shop Yard - On or about January 16, 2013 employees were exposed to bench grinder that had the work rest adjusted to three fourths inches from the wheel.

29 CFR 1910.215(b)(9):   Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch:  (a) Shop yard - On or about January 16, 2013 a dual bench grinder lacked tongue guards exposing employees to a struck by hazard.
