29 CFR 1910.146(c)(8)(v):  When an employer arranged to have employees of another employer (contractor) perform work that involved permit space entry, the host employer did not debrief the contractor at the conclusion of the entry operations regarding the hazards confronted or created in permit spaces during entry operations:  a)  Suncor Energy, Inc. at 5801 Brighton Blvd., Commerce City, CO 80022:  On or before October 29, 2012, the employer authorized Safway Services, LLC to erect and modify scaffold in the permit space inside Heater 401, including the erection of 3 outrigger scaffolds that presented fall hazards to entrants.  Suncor did not debrief Safway Services, LLC regarding the fall hazards created during the erection and modification of this scaffold.  In addition, other contractors used this scaffold, including pipefitters, welders, and inspectors, and Suncor did not debrief these contractors regarding the hazards confronted during entry operations.  Condition exposed welders, pipefitters, and inspectors to fall hazards of up to 25 feet.

29 CFR 1910.146(f):  The entry permit that documented compliance with this section and authorized entry into a permit space did not identify the requirements in (f)(1)-(15) of this section:  a)  Suncor Energy, Inc. at 5801 Brighton Blvd., Commerce City, CO 80022:  On or before October 29, 2012, the employer approved permits for subcontractor use in entering Heater 401, even though the permits did not identify the requirements of (f)(1) - (f)(15) in the following instances:  1)  the authorized entrants within the permit space (Mistras permits # DN1-0000333920 and # DN1-0000334553);  2)  the personnel, by name, currently serving as attendants (Misras permits # DN1-0000333124, DN1-0000333920, and # DN1-0000334553);  3)  the entry supervisor by name, with a space for the signature or initials of the entry supervisor who initially authorized entry (all Mistras permits);  4)  the hazards of the permit space to be entered (all Mistras permits);  5)  the measures used to isolate the permit space and to eliminate or control permit space hazards before entry (all Mistras permits);  6)  the acceptable entry conditions (all Mistras permits);  7)  the results of the initial and periodic air monitoring tests, performed under (d)(5) of this section, accompanied by the names or initials of the testers and by an indication of when the tests were performed (all Mistras permits);  8)  the communication procedures used by the authorized entrants and attendants to maintain contact during entry (all Mistras permits); and  9)  equipment, such as personal protective equipment, testing equipment, communications equipment, alarm systems, and rescue equipment to be provided for compliance with this section, including the following Mistras permits:       a)     Permit # DN1-0000333124 for Heater 401 dated October 25, 2012 showed N/A for  Rescue/Fall Protection;        b)     Permit # DN1-0000333920 for Heater 401 dated October 26, 2012 showed N/A for  Rescue/Fall Protection;        c)     Permit # DN1-0000334553 for Heater 401 dated October 27, 2012 showed N/A for  Rescue/Fall Protection; and         d)     Permit #DN1-0000335606 for Heater 401 dated October 29, 2012 showed N/A for  Rescue/Fall Protection.   Conditions exposed employees to fall hazards of up to 25 feet.
