29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a)  On or about January 9, 2013, employees were exposed to fall hazards in excess of 10 feet, while installing a new roof without fall protection. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) On or about January 9, 2013, employes were exposed to fall hazards while using an extension ladder that did not extend at least 3 feet above the upper landing surface nor was the ladder secured in place to prevent accidnetial displacement. 29 CFR 1926.1053(b)(6): Unsecured ladders were used on surfaces that were not stable and level:    (a) On or about January 9, 2013, employees were exposed to fall hazards while using an unsecured extension ladder to access the roof area.  The ladder was placed on a slope of dirt and the footing of the left side rail was not engaged.
