OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and fall from elevation hazards relating to improperly braced trusses:      a) On 01-15-13, employees who were installing trusses with sixty (60) foot spans for a new addition to an existing commercial building did not install the required temporary lateral, diagonal, and web bracing; employees were exposed to struck by and fall from elevation hazards associated with the improperly braced trusses.    Among other methods, one feasible and acceptable means to correct these hazards is to follow the Building Component Safety Information (BCSI-2008) guidelines for temporary bracing.  This information was available on site.       ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1926.501(b)(15):     Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:    a)  On 01-15-13, an employee who was installing trusses was walking on the wall top plate at a height greater than fifteen and one half (15.5) feet above concrete surface without utilizing fall protection equipment.    b)  On 01-15-13, an employee who was installing trusses was walking on the bottom cord of trusses at a height greater than fifteen and one half (15.5) feet above concrete surface without utilizing fall protection equipment.        ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      a) On 01-15-13, an employee who was installing trusses was utilizing a twenty-eight (28) ft. extension ladder to access/egress a wall top plate working surface greater than fifteen and one half (15.5) feet above concrete surface, without any grab rails extending above the upper landing surface.    NO ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1926.1425(c)(3): The materials were not rigged by a qualified rigger:      a) On 01-15-13, an employee, who was not a qualified rigger, was rigging Alpine ITW Building Components Group Inc. sixty (60) foot long trusses to wire rope slings on a crane by using two web slings; each web sling was located on one side of the center point of the truss, looped around the top cord at a web connection at a distance greater than nine (9) feet from the truss center ridge point.
