29 CFR 1926.451(b)(4): Each end of a platform, unless cleated or otherwise restrained by hooks or equivalent means, shall extend over the centerline of its support at least 6 inches (15 cm). On or about January 15, 2013 at 550 South Locust Street, Oxford, at an interior and exterior renovation of an existing building, a masonry employee was exposed to a fall hazard while bricklaying from the cantilever of a welded frame scaffold with a wood scaffold plank extending less than six inches past the centerline of the support.

On or about January 15, 2013, at 550 South Locust Street, Oxford, during an interior and exterior renovation of an existing building, a masonry employee was exposed to a fall hazard while bricklaying from the cantilever of a welded frame Biljax scaffold due to the lack of base plates on the outer legs of the scaffold.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. On or about January 15, 2013, at 550 South Locust Street, Oxford, at an interior and exterior renovation of an existing building, a competent person did not inspect a welded frame scaffold before the work shift that was being used by a mason employee that day to brick a part of the edge of a loading dock door opening due to the outer scaffold legs were missing base plates and one of the wood scaffold plank did not extend at least six inches from the centerline of the support on each end.
