29 CFR 1910.132(a): Employer did not provide personal protective equipment to be used when necesary whenever hazards capable of causing injury or impairment were encountered.  a) On or about January 14, 2013: In bay #4, the employer did not provide fall protection for an employee that was on a flatbed truck unloading 12" SQ x 1/4 x 48' square tubing load using a 10 Ton Bridge Crane 70'. The employee was on top of the material hooking the chains on to the lift when he slipped and fell approximatelly 7 1/2 feet to the ground.
