Three of the four Class B fire extinguishers were not charged rendering them unusable.

29 CFR 1910.305(a)(2)(xi): Cable assemblies and flexible cords and cables were not supported in place at intervals that ensure that they will be protected from physical damage: Employees working at the drilling rig located at SEC 28, TWP 11, R 17, 990 fls, 330 fwl near Hays, Kansas were exposed to electrical shock hazards. The electrical power cable for the drilling rig was laid across the ground exposing it to physical damaged from vehicle and foot traffic.

The 450 volt AC main electrical breaker panel mounted on the generator trailer was missing the entire front cover exposing live electrical components.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Employees working at the drilling rig located at SEC 28, TWP 11, R 17, 990 fls, 330 fwl near Hays, Kansas were exposed to electrical shock hazards. The 120 volt extension cord used to power a heater had the plug pulled away from the cord exposing the insulated wires.
