29 CFR 1910.132(a):     Protective equipment was not maintained in a sanitary and reliable condition:    On or about January 16, 2013 and times prior thereto, employer failed to maintain personal protective equipment in a reliable condition.  On the Komatsu forklift in the Die Cast area the seat belt had been removed exposing drivers of the equipment to ejection hazards in the event of a roll-over or quick maneuver.

29 CFR 1910.212(a)(3)(ii):     Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    On or about January 16, 2013 and times prior thereto, the point of operation guards in the following instances were not designed and installed to keep the operators from reaching the danger zone during the working cycle of the machine subjecting them to amputations.    instance a.) The interlocks on the air operated sliding doors on the die cast machines are being defeated by being wired down.    instance b.) The two hand controls on the Dennison Hydraulic Trim Press are not configured to prevent the tying down of one of the controls.

29 CFR 1910.303(c)(3)(i):     Conductors shall be spliced or joined with splicing devices identified for the use or by brazing, welding, or soldering with a fusible metal or alloy. Soldered splices shall first be spliced or joined to be mechanically and electrically secure without solder and then soldered. All splices and joints and the free ends of conductors shall be covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose.    On or about January 16, 2013 and times prior thereto, the power cord for a portable pedestal fan in the die cast area had its cord spliced together and covered with tape.  The insulating value of the covered joint could not be determined to be equivalent to that of the conductors.  Employees handling the spliced joint could be exposed to the hazards of an electrical shock.

29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    On or about January 16, 2013 and times prior thereto the employer failed to perform a visual inspection and remove from service an electrical cord and plug that was used to power a portable wooden box fan.  The power cord had breaks in its outer insulation which exposed the conductors to damage.
