29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about January 17, 2013, at this construction site, an employee was doing window glass installation work while standing on a square tube steel metal beam approximately eight feet six inches high above ground level without fall protection.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: On or about January 17, 2013, at this construction site, an employee was climbing off on of a self supporting step ladder from the side onto a beam approximately nine feet above the ground. This condition exposed the employee to a fall hazard.
