29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: On or about January 17, 2013, employees performing masonry activities at a residential property were exposed to slips, trips and fall hazards from material such as but not limited to bricks, stone, lumber, holes and scaffold parts.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. On or about January 17, 2013, employees performing masonry work on a residential home using a scaffold system did not have all open ends and sides of each working platform guarded by guardrail systems. Employees were exposed to the hazard of falling.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall. On or about January 17, 2013, employees performing masonry activities on a residential home were working from upper levels of a tubular scaffold system without toe boards or protective devices to protect the employees working at lower levels. Employees were working at upper levels with bricks, large stones, tools and other equipment exposing employees at lower levels to being struck by falling objects.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: On or about January 17, 2013, employees performing masonry activities on a residential home were using a 6 foot A-framed ladder folded and tied to the scaffold. Employees were climbing the ladder for access and to exit the scaffold. The upper platform was approximately 7 feet from the ground.
