29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.    The employer does not provide employees with written energy control procedures for conducting lockout/tagout activities.  This violation was observed on or about January 15, 2013, in the Robotics Area, Thermal Spray Coatings where employees were exposed to electrical, caught-between and struck-by hazards while performing service and/or maintenance on machines or equipment without written procedures.      Pursuant to 29 C.F.R. 1903.19, within then (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the energy control program includes written specific energy control procedures to ensure employees performing any servicing or maintenance on a machine where stored energy is found is protected.

29 CFR 1910.212(a)(1):  Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     The employer does not guard the point of operation of machines whose operation exposes an employee to injury:     a)  This violation was observed on or about January 15, 2013, in the Grinding Room where employees were exposed to a stuck-by hazard when operating Grinder (#2, Serial #7041) without a chuck guard.     b) This violation was observed on or about January 15, 2013, in the Grinding Room where employees were exposed to a struck-by hazard when operating a Heald Lathe (Model 274) without chuck guard.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including the steps that it is taking to ensure that guards are installed on all machines to protect employees from the point of operation.

29 CFR 1910.304(f)(1)(iv):  Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel.    The enployer does not ensure that electrical devices and panels are readily accessible without being blocked by machinery and/or storage of equipment.  This violation was observed on or about January 15, 2013, Grinding Department, where employees had stored in front of JC4 Breaker Panels and Circuit Breaker Boxes brooms, equipment and metal tool boxes not allowing ready accessibility.     Pursuant to 29 C.F.R. 1903.19, within 10 (ten) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees have ready access with 3 foot of clearance around circuit breaker boxes and/or panels.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.    The employer does not ensure that strain reliefs on flexible cords and cables were not damaged.  This violation was observed on or about January 15, 2013, the strain relief on a cord and plug set that was attached to a spot light was damaged, exposing the inner insulated wires .    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that strain reliefs on flexible cords and cables are installed properly without exposing inner insulated wiring.
