29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by the employees: a) Plant - On or about 01/15/13 workers operated, maintained and serviced equipment including (but not limited to) the 8.0 ft. 4.0 in. saw, the 11.0 ft. 6.0 in. saw, the 5.0 ft. 4.0 in. saw, the 36 in. saw, the 44 in. saw, the tip saw, the profile saw, and the Hydrasplit stone splitter. Employees were exposed to electrical and hydraulic energy, and the employer did not provide training in reference to the control of hazardous energy and the lockout/tagout program. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts: a) Plant - On or about 01/15/13 workers operated the 8.0 ft. 4.0 in. circular saw and the machine was not guarded. b) Plant - On or about 01/15/13 workers operated the 11.0 ft. 6.0 in. circular saw and the machine, and the area surrounding the blade was not guarded. d) Plant - On or about 01/15/13 workers operated the 5.0 ft. 4.0 in. circular saw, and the area surrounding the blade and the machine was not guarded. e) Plant - On or about 01/15/13 workers operated the 36 in. circular saw, and the area surrounding the blade was not guarded. f) Plant - On or about 01/15/13 workers operated the 44.0 in. circular saw, and the area surrounding the blade was not guarded. g) Plant - On or about 01/15/13 workers operated the 24.0 in. circular tip saw, and the area surrounding the blade was not guarded. h) Plant - On or about 01/15/13 workers operated the profile saw, and the area surrounding the blade was not guarded. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
