29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents. Employees were not provided eye protection while using nail guns and saws during the installation of cedar shake shingles. Employees were exposed to a struck-by hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: Employees performing roofing operations on the roof with a 9/12 pitch were not utilizing fall protection. Employees were exposed to a fall hazard of approximately nine (9) feet eave to ground.
