29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  Employees were not provided eye protection while using nail guns and saws during the installation of cedar shake shingles. Employees were exposed to a struck-by hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Employees performing roofing operations on the roof with a 9/12 pitch were not utilizing fall protection. Employees were exposed to a fall hazard of approximately nine (9) feet eave to ground.

29 CFR 1926.503(c)(3): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee.   Employees performing roofing operations had not been trained to recognize and minimize the hazards of working aloft.
