29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: On or about January 15, 2013, at the above mentioned jobsite, fours employees were using a ladder to access a roof to conduct roofing work without the ladder being secured. THIS VIOLATION REQUIRES ABATEMENT CERTIFICATION.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about January 15, 2013, at the above mentioned jobsite, four employees were on a residential roof conducting roofing work without a means of fall protection at a height of approximately 9 feet to the ground. T & S ROOFING SYSTEMS INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501 (B)(10), WHICH WAS CONTAINED IN OSHA INSPECTION 314260837, CITATION NUMBER 1, ITEM NUMBER 5, AND WAS AFFIRMED AS FINAL ORDER ON JULY 28, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 1300 NE 191ST STREET, NORTH MIAMI BEACH, FL 33179. CERTIFICATION AND DOCUMENTATION OF ABATEMENT ARE REQUIRED FOR THIS VIOLATION.
