29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:    Pre Touch Booth, Left Side - On 2/19/13 an employee doing powder coating painting with Bronze Metallic Super Polyester IH Bonded Semi Gloss wearing a 3M dust respirator #8577, P95 was exposed to 5.5 times the OSHA PEL of 1 mg/m3 for Copper Dust as Copper as an 8-hour Time Weighted Average.  The TWA of 5.5 mg/m3 was calculated from 195 minutes exposure and 285 minutes of zero exposure.  The 3M dust respirator has an assigned protection factor of 10 provided the employer was fit tested.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Copper Dust as Copper listed in Table Z-1 in excess of the 8-hour Time Weighted Average concentration of 1 mg/m3:     Pre-Touch Booth Left Side - On 2/19/13 an employee doing powder coating painting with Bronze Metallic Super Polyester IH Bonded Semi Gloss was exposed to 5.5 times the OSHA PEL of 1 mg/m3 for Copper Dust as Copper as an 8-hour Time Weighted Average (TWA).  The TWA of 5.5 mg/m3 was calculated from 195 minutes exposure and 285 minutes of zero exposure. Health effects of Copper are primarily irritation of eyes, nose, throat and skin.
