29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a) On or about 01/10/2013 employees were observed working from tubular welded scaffolds and open side platforms with no guardrails at the construction of the Courtney at Bay Pines Apartments in Seminole Florida. The employees that were exposed to falls of approximately 25 feet to the ground were not instructed on the correct procedures of the fall protection system to be used.

The I&A Construction (DBA as Iris Construction) was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 316306687, citation number 1, item number 3 and was affirmed as a final order on 3/12/2012, with respect to a workplace located at Lake Mary, Florida.

The I&A Construction (DBA as Iris Construction) was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 626318, citation number 1, item number 3 and was affirmed as a final order on 10/22/12, with respect to a workplace located at Riverview Florida.
