29 CFR 1910.22(b)(1): Where mechanical handling equipment is used, sufficient safe clearances were not allowed for aisles, through doorways and wherever turns or passage must be made. Aisles and passageways were not kept clear, with no obstruction across or in aisles that could create a hazard. On or about December 17, 2012, and times prior thereto warehouse aisles, and doorways were not kept clear of obstructions. These conditions exposed employees to powered industrial truck struck-by hazards and fire potential hazards.

29 CFR 1910.157(c)(1): The employer did not have a portable fire extinguisher readily accessible to employees without subjecting the employees to possible injury. On or about December 17, 2012, a portable fire extinguisher located in the east wall of the warehouse was obstructed as to prevent access. This condition exposed employees to trip and fall hazards.

29 CFR 1910.333(b)(2): Employees exposed to contact with parts of fixed electric equipment or circuits which have been de-energized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph. On or about December 17, 2012, a fix electrical equipment such as a step-down transformer and control panel with exposed contact parts (Knock-outs open; wires exposed to abrasion) which have been de-energized, the circuits energizing the parts were not locked and tag-out. These conditions exposed employees to electrical hazards.
