29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. (a) Maintenance: On 01/10/2013 and at times prior thereto, workers were subject to slip/trip/fall injuries due to cartons and cords on the floor, boxes protruding from shelving, and poorly stacked items on shelves. (b) Insulation: On 01/10/2013 and at times prior thereto, workers were exposed to a fire hazard due to the build-up of paper-based insulation material on horizontal surfaces above the work area. (c) Windows: On 01/10/2013 and at times prior thereto, workers were exposed to trips and falls due to dust, tools and palletized materials in aisles. 29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: (a) Station 11: On 01/10/2013 and at times prior thereto, workers were exposed to falling from an open-sided walkway due to a slack mid-rail cable positioned 26.5 inches below the top rail and 13.5 inches from the walking surface before any pressure was applied to the cable.
