29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: A.) Hanna Transfer Company dba Moonlight Packaging Building A; Exit door C on east side of building - On or about 1/18/13, exit doors were locked while employees were in the building performing work activities. This door was locked with a turn latch which was keyed on the outside of the building. The door could not be opened when unlocked. This door was marked as an exit. B.) Hanna Transfer Company dba Moonlight Packaging; Building B; Exit door in storage area on north side of the building - On or about 1/18/13, exit doors were locked while employees were in the building performing work activities. This door was locked with bars over the window and it was bolted shut and had a slide lock in place. C.) Hanna Transfer Company dba Moonlight Packaging; Building B; Exit door C on the east side of the building - On or about 1/18/13, exit doors were locked while employees were in the building performing work activities. This door was locked with a slide lock. D.) Hanna Transfer Company dba Moonlight Packaging; Building B; Exit door B on the west side of the building - On or about 1/18/13, exit doors were locked while employees were in the building performing work activities. This door was lock with a bungee cord being attached to the panic bar. This door was also blocked by material storage in front of it.

Employees performed servining and maintenance operations on machinery including, but not limited to, the 36 inch press, the 60 inch press, and the stitcher machine. Maintenance and servicing operations included but were not limited to, set up, repairs, and preventative maintenance.
